KINDERGARTEN ENROLLMENT
Parents of kindergarten students residing in the Lakeside School District with a student five years of age on or before August 1, 2012, may begin online. If you choose to waive kindergarten enrollment for the 2012-2013 school year please complete and submit a Kindergarten Waiver. (This form can be found below)
If you Do Not reside in the Lakeside School District, please visit our information on School Choice. (Click on the link below.) This will get you all the information you need. If you are accepted, you will need to bring your acceptance letter as proof during registration.

The online registration is a pre-registration form only and students will not be considered enrolled until parents provide the completed pre-registration packet and the documents listed below to the Primary School's office.
For the parents of children currently residing in Lakeside School District and currently enrolled in the Lakeside School District Pre-K program, parents must complete and provide the kindergarten packet and a new proof of residency before the student is enrolled at Lakeside Primary.
REQUIRED DOCUMENTS
Print and bring the pre-registered application forms to the Primary Office.
Proof of residency (one of the following)
Copy of current electric or water bill showing the service address and the parent's name and address.
The rent or lease agreement with the residential address, name and contact number of the homeowner.
Copy of the child's birth certificate.
Copy of the child's social security card.
A current physical examination
Papers concerning legal guardianship (if applicable)
Up to date immunization records. The following immunizations are required:

NOTE: All required documents must be received by August 1st for your child to be assigned to a classroom.
