LAKESIDE PRIMARY SCHOOL

PRE-KINDERGARTEN ENROLLMENT

 Parents of Pre-K students residing in the Lakeside School District with a student four years of age on or before August 1, 2012, may begin online pre-registration. 

NOTE:  Students that reside in the Lakeside School District are given the first available slots to attend Lakeside Pre-K.

Parents of Pre-K student that do not reside in the Lakeside School District may also complete the re-registration forms and must contact the Primary Office to be put on our Pre-K waiting list beginning May 2, 2011.  The Primary Office will begin filling the final spots for Pre-K, from the students on the waiting list beginning July 27, 2012.

The online registration is a pre-registration form only and students will not be considered enrolled until parents have completed the pre-registration packet and provided the documents listed below to the Primary School's office.

NOTE: Parents will be required to bring the information below to the Primary School's office in order to formally complete the enrollment of their child.  Dawson Cooperative will be screening the Pre-K students at Lakeside Primary. Date will be posted soon.

REQUIRED DOCUMENTS

1. Print and bring the pre-registered application forms to the Primary Office.
2. Proof of residency (oneof the following)
   • Copy of current electric or water bill showing the service address and the parent's name and address.
   • The rent or lease agreement with the residential address, name and contact number of the homeowner.
3. Copy of the child's birth certificate.
4. Copy of the child's social security card.
5. A current physical examination
6. Papers concerning legal guardianship (if applicable)
7. Proof of income. (pay stubs or W-2 form)
8. Up to date immunization records.  The following immunizations are required:

NOTE:  All required documents must be received by August 1st for your child to be assigned to a classroom.
